How to Apply:
Via Online Form or Fax: Fill out form and click submit below or Print and Fax application to fax (1 207 342 5768) and then call to place deposit on credit card
Via Mail: Print and Mail application and check to CSJ's office, address below. Please make checks payable to "SCOTT JONES"

NOTICE: Deposit Required for Reservation. The condo will only be reserved when deposit is received.

Please select from one of the following options:
I choose to pay a 20% deposit on my credit card over the phone (1 888 345 6077).
I choose to mail a 20% deposit check to CSJ's office (State of Maine). I will mail the full and final payment to CSJ's office (State of Maine), in order to arrive one month before arrival.

Initial I understand my condo time is not reserved until my deposit check and my application are received, and I have been notified by email, fax or telephone.

Security Deposit Required.

I acknowledge that C.S.J. has my credit card number on file, and my authorization to use this, in case of any damages to the condo which are not the result of normal wear and tear.

I will mail a check for $500 to C.S.J.s main office. This check is due four weeks prior to arrival day and the start of use of the condo. Deposit check will be returned upon departure minus any damages to the condo which are not the result of normal wear and tear.
