How to Send an Application to C.S.J.

Space on the trip is reserved when C.S.J. has received an application and $500 deposit per person.

You may use any one of the following procedures to send an application to CSJ:

Email: Fill out the application form on our website. Simply type your personal data directly on the application. Then click on the Apply via Email button to email it directly to us. You may then either mail us your deposit check or call us with your credit card number.

Fax: Fill out the application form on our website, then click on the Print This Page button. Fax it to our phone fax 1 207 342 5768. Alternatively you may print the page first, and fill out the form by hand before faxing. You may then either mail us your deposit check or call us with your credit card number.

Postal Mail: Fill out the application form on our website, then click on the Print This Page button. Mail the printed application to the address listed on the bottom of the page. (C.S.J. / 28 Morrill Rd. / Montville, ME 04941). Alternatively you may print the page first, and fill out the form by hand before mailing. You may either mail us your deposit check along with the application form or call us with your credit card number.
